Leonard Joseph Dvorak, 83, of Toledo, passed away Tuesday, November 19, 2019, at the Iowa Lutheran Hospital-UnityPoint in Des Moines. A Memorial Visitation will be held Friday, November 22, 2019, from 5-7 PM at Kruse-Phillips Funeral Home in Tama. Private Interment will be held at Stayskal Cemetery in rural Vining, Iowa, at a later date.

Survivors include one daughter, Sheryl (Ken) McMurray, of Tama; two grandsons; two great-granddaughters; one sister-in-law, Libby Dvorak, of Chelsea, and several nieces, nephews and cousins.

A memorial fund will be established.
